Become a Member of the SLDA

There are two ways to become a Member of the SLDA. You may download the membership application form here and send it printed and duly filled along with payment to;

Alternatively, you may fill the online form below to regsiter with SLDA. Make sure you have your Medical Council Registration Number and names of two (2) referees who are members of the SLDA. You will need to have an email address so we can deliver information about your registration status.

Online Application Form

Any person duly registered in the Dental Register of Sri Lanka Medical Council (SLMC) is eligible for membership of the SLDA.
Acceptance of your Membership will be communicated to you after the next council meeting that will be held following the payment for acceptance of membership.
An Ordinary Member should make payment before December 31st to continue Membership without loss of seniority.

WI-FI ENABLED

The SLDA office has become a Wi-Fi enabled zone. Any member could utilize the privilege of internet access free of charge by bringing their own laptops to the SLDA office and connecting to the net.
